We love you guys. It’s the only time I’ve ever had a pet that looked forward to going to the vet. He loves all the attention, care and treats he gets when he comes here....

All animals staying at University Animal Hospital must be current on their vaccinations. If your pet is not up to date, we can take of it when they come in.

Dogs must be current on their Distemper/Parvo, Rabies and Bordetella (kennel cough) vaccines. The Bordetella must have been administered in the last 6 months. We also require that all dogs be treated with a flea/tick treatment such as Advantix or Frontline, this can be administered at home or when they come in.

Cats and ferrets must be current on their Distemper and Rabies vaccines.

Please note that all boarding fees are increased over major holidays by 15%.

As the owner or authorized guardian of this animal, I give permission to University Animal Hospital to receive, treat, prescribe or otherwise care for the animal above as deemed necessary. Should injury or a life or death emergency occur we will need to reach you or someone you delegate to make medical and financial decisions for your pet. If we are unable to reach anyone we will act in the best interests of your pet. This may mean surgery, diagnostics etc. and these procedures can cost thousands of dollars. Costs will likely exceed the amount you have "checked off" on your boarding paperwork. IF YOU DO NOT WANT LIFE SAVING MEASURES GIVEN TO YOUR PET YOU MUST NOTIFY US AT THE TIME OF DROP OFF. We will have you sign a separate form.

I understand that University Animal Hospital will try to contact the necessary people before treatment, will exercise the option to proceed if no one is available for approval and I will be charged the normal, customary fees. Payment will be due in full at the time I pick up my pet.
